Each aspiring student who joins our community will train under the guidance of Shifu Shi Yanjun, a 34th-Generation Shaolin warrior monk and 17th-Generation Inheritor of the Meihua Quan tradition, a living embodiment of China’s Intangible Cultural Heritage.

We kindly request that you complete the application form with accuracy and integrity. Your honesty is valued and appreciated.

Incomplete or inaccurate applications will not be considered.

Please indicate your nationality as per your passport. Racial or ethnic background is not required for visa processing.

Waiver and Release of Liability

I, the undersigned, understand that intensive Kung Fu training involves significant physical exertion and carries inherent risks of injury. I hereby release China Intangible Cultural Heritage Xingtai Meihua Quan, its instructors, and staff from any and all liability for injuries I may sustain while participating in classes or school activities. I have read and understand this waiver and release of liability. I am aware that I am responsible for my health insurance and any medical expenses incurred during my training in China.
